Patient engagement has been a central and required component of projects funded through MeHAF’s Integration Initiative. The purpose of this case study is to describe various approaches to patient engagement while providing integrated care so other providers to learn from and adopt these strategies. The case describes the work of five diverse organizations: Amistad; Pen Bay Healthcare; Sacopee Valley Health Center; Spring Harbor; and Tri-County Mental Health Services – and the Maine Patient Centered Medical Home Pilot, a coalition of organizations working across the state to implement medical home principles, led by Quality Counts, a statewide quality improvement organization. These organizations represent the diverse types of health care providers in Maine, and their approaches and emphasis on engagement activities differ based on the organization’s setting and local priorities.
